I/We request you to please cancel the above-mentioned instrument(s) and accordingly mark the instrument(s) as CANCELLED in your records and refund the amount thereof to me/us as it is no more required.

I/We hereby authorize the Bank to deduct from my/our account service charges; or agree to pay unconditionally, by authorizing the bank to recover from me/us, the service charges, as the case may be, in relation to cancellation of instrument(s) as per Bank’s schedule of Charges.

I/We also hereby indemnify and hold Bank, its officers and employees harmless from and against any and all losses, damages, liabilities, payments and obligations and all expenses (including without limitation reasonable legal fees) incurred, suffered, sustained or required to be paid, directly or ought to be imposed upon Bank by any Court of Law or any other authority, as a result of cancellation of instrument(s).

Furthermore, I/We undertake and agree that if any claim is made by the beneficiary against any such instrument(s); I/We shall be fully responsible to make payment there against. Therefore, I/We am/are bound to pay the amount of the instrument(s) to Bank on first written demand of the Bank, immediately without any prejudice and recourse, if any such claim arises and payment is required to be made.

I/We am/are executing this Undertaking/Indemnity in favor of the Bank on this 09 day of January 2026, here at Islamabad.

LETTER OF UNDERTAKING FOR GENUINENESS OF THE BENEFICIARY DISCHARGE ON PAYMENT INSTRUMENT

This Letter of Undertaking is made / executed at Islamabad on this 09 day of January 2026.

Attached please find herewith the original Payment Order/Demand Draft # 814400134 for the amount of 173,250.00 dated 01 September 2025 favoring Executive Director, Punjab Institute of Mental Health, Lahore issued by Branch I-8 Markaz Islamabad at my/our request.

I/We, Najeeb Ullah Khan bearing CNIC/NTN # 61101-7811812-9, fully endorse the genuineness discharge by the beneficiary on the Payment Instrument and if any claim is made by the beneficiary of the said Payment Instrument subsequent to the cancellation thereof, the Bank shall, without notice to me/us, be entitled to debit the amount of the Payment Instrument from my/our account.

I/We undertake & agree to indemnify Al Baraka Bank (Pakistan) Limited, each of its administrators and successors, employees, Officers, Executives, Directors, Shareholders from & against any or all claims, rights, remedies, costs, expenses, or proceedings of whatsoever nature arising (if any) sustained or incurred directly or indirectly as a result of cancellation / refund of the aforementioned Payment Instrument against the discharge of the beneficiary and do hereby undertake to pay all such claim, damages, losses, cost and expenses and do hereby authorize the Bank to debit our account we have at any if its branches to recover all such claims, damages, losses, and expenses.
